Abstract
Objective:To compare the intervention effects of different traditional Chinese medicine (TCM) methods in patients with delayed anal exhaust after gastrointestinal surgery.Methods:94 patients who underwent gastrointestinal surgery in our hospital between April 2014 and August 2015 were selected,and all patients had symptoms of delayed anal exhaust (>3d).The patients were randomized into 3 groups and were given different interventions.30 patients in the controlled group were only given condition monitoring after surgery,incision cleaning and other routine nursing interventions;The 32 patients in TCM intervention group ①,on the basis,were given Tongfu Paiqi Decoction,150ml/once,twice/day;On the basis of the controlled group,the TCM intervention group ② were given warm water lavipeditum massage,soaking 25min/once,3 times/day.While soaking,foot acupoint massage was performed.5 days of continuous intervention later,the effects of interventions in the 3 groups on gastrointestinal dynamics [motilin (MTL),gastrin (GAS),serotonin (5-HT)] in patients with delayed anal exhaust were observed.The occurrence of abdominal distension and pain,nausea and vomiting,low-grade fever and other related gastrointestinal symptoms in the two groups was recorded.The changes of anal exhaust time and defecation time after intervention,scores of visual analogue scale (VAS) and quality of life (SF-36) before and after intervention were compared between the 3 groups.Results:(1) In terms of gastrointestinal dynamics,MTL,GAS and 5-HT in TCM intervention group ① and ② were significantly improved,compared with those before intervention.Besides,MTL and 5-HT were better than those in the controlled group (P < 0.05).However,there were no significant differences between the two groups (P > 0.05);(2) In terms of gastrointestinal symptoms,the incidence rates of gastrointestinal adverse symptoms in TCM intervention group ① and ② were significantly lower than those in the controlled group (P < 0.05),but there were no significant differences between TCM intervention group ① and ② (P > 0.05);(3) In terms of exhaust and defecation,the anal exhaust and defecation time of TCM intervention group ① and ② was shorter than that of the controlled group (P < 0.05),but there were no significant differences between TCM intervention group ① and ② (P > 0.05);(4) In terms of VAS and SF-36,VAS scores of TCM intervention group ① and ② after intervention were lower than those of the controlled group while SF-36 score was higher than that of the controlled group (P < 0.05).The VAS score of TCM intervention group ② was lower than that of TCM intervention group ① (P < 0.05).Conclusions:①To apply Tongfu Paiqi Decoction and warm water lavipeditum massage in patients with delayed anal exhaust after gastrointestinal surgery can effectively improve gastrointestinal notility,promote gastrointestinal peristalsis,reduce gastrointestinal symptoms and shorten the time of anal exhaust and defecation.It also can significantly relieve pain and improve the prognosis quality;②Warm water lavipeditum massage has more advantages over Tongfu Paiqi Decoction in relieving pain and it is more suitable for the ederly patients and intolerance patients.关键词
