Abstract
Objective:To explore the possibility of using maternal characteristics plus multiple routine blood indexes to predict gestational diabetes mellitus earlier in the pregnancy.Methods:Retrospectively analyzed 8513 cases of pregnant women(excluded diabetes pregnant women in 234 cases) who sought their first antenatal care in the First Maternity and Infant Hospital of Shanghai between Jan.2013 and Dec.2014.The data including maternal characteristics and blood test results at 11~18 gestational weeks.They were divided into 2 groups(GDM group,n=1087;Normal group,n=7426),according to the result of 75g oral glucose tolerance test in their 24~28 gestational week.Results:There were differences in age,body mass index,white blood cell count,hemoglobin,alanine aminotransferase,serum ferritin,thyroid stimulating hormone,fasting plasma glucose,glycosylated hemoglobin and hepatitis B virus e antigen between the two groups.Using these indexes to established the predictive model of GDM,the accuracy was 74.79% at the false positive rate of 46%.The formula was used to prospectively predict the development of GDM in 100 cases of pregnancy women in their 11~18 gestational week,the accuracy was 81.25% as the false positive rate was 57.14%.Conclusion:Using the maternal characteristics combined with multiple routine blood indexes in early pregnancy can effectively predict gestational diabetes mellitus.By strengthening the early education of the prediction positive pregnant women,maybe the incidence of gestational diabetes mellitus can be reduced.关键词
