伏立康唑治疗COPD合并肺真菌感染的不良反应分析

Abstract
Objective To characterize adverse reaction induced by Voriconazole in treating patients with both acute exacerbation chronic obstructive pulmonary disease (AECOPD) and lung fungal infection.Methods Total 150 patients with AECOPD and FLI admitted into the Dongfeng Hospital of Hubei University of Medicine from January 2013 to April 2016 were included in the study,32 patients were treated with Voriconazole injection.Results Among patients subjected to Voriconazole treatment,8 cases (25％) exhibited adverse reactions with presence of psychonosema in 4 cases.A total of 17 times of adverse reactions were reported in these 8 cases.The major reactions included visual impairment,skin rash,liver and kidney dysfunction,hypokalemia,mental disorders,urinary incontinence.Multiple reactions were found in each subject among 50％ cases.Multiple systems and organs were involved in the VAR which occurred at no specific time point during treatment.Psychonosema and visual disturbances appeared earlier,and Psychonosema appeared 2～3 days after treatment in 4 cases.In addition to Voriconazole,medications,including levofloxacin,methylprednisolone,phenergan,insulin,statins,pantoprazole,mosapride were applied to 8 subjects.All adverse reactions gradually disappeared after drug withdrawal.All patients were cured with no sequela or death.Conclusion Voriconazole induced adverse reaction in treating patients with beth AECOPD and lung fungal infection.Psychonosema was an indicator for drug withdrawal or dosage adjustment.关键词
