速度向量成像技术评价慢性肾脏疾病患者左房功能和运动非同步性研究
Assessment of left atrial dysfunction and asynchrony in patients with chronic kidney disease by velocity vector imaging

Abstract
Objective:To investigate the changes of left ventricular (LV) structure and function,left atrial (LA) function and synchrony in patients at different stages of chronic kidney disease (CKD) using ultrasonic velocity vector imaging (VVI).Methods:CKD patients treated in our hospital from January 2013 to August 2016 were recruited.According to the estimated glomerular filtration rate (eGFR),patients were divided into CKD stage 2 3 group (group A,n=52) and stage 4 5 group (group B,n=51).Age and sex matched healthy volunteers were served as control group (group C,n=50).LV wall thickness and function were measured by echocardiography.LA peak systolic strain rate (SR-s),early diastolic strain rate (SR-e),late diastolic strain rate (SR-a),standard deviation of the time to peak longitudinal strain rate (SD-Tsr) of six segments,and the maximal difference of the time to peak longitudinal strain rate between any two segments (Max-Tsr) were analyzed.Results:SR-s and SR-e reduced while LV wall thickness,LV mass index and E/e'increased gradually with the progression of CKD.SR-a was increased in group A,while decreased in group B.SD-Tsr and Max-Tsr in groups A and B were increased significantly compared with the control group,which were more obvious in group A than in group B.Conclusion:The LV structure and function,as well as the LA function and synchrony were changed in patients with CKD.VVI technology could be used to evaluate the early changes of LA function and synchrony in CKD patients.关键词
