乳腺癌原发灶弥散加权成像在预测腋窝淋巴结转移中的应用价值
Predictive value of diffusion weighted imaging of primary foci in cases of breast cancer with axillary lymph node metastasis

Abstract
Aim:To evaluate the diagnostic value of magnetic resonance diffusion weighted imaging(DWI) of breast cancer primary foci in the patients with axillary lymph node metastasis.Methods: A total of 33 patients with breast cancer admitted in our hospital from January 2014 to June 2015 were selected,who were confirmed as breast infiltrating ductal carcinoma by pathology.DWI and pathology information of the patients were retrospectively analyzed,including the lesion size,signal uniformity,ADC values,the length of axillary lymph node,lymphatic vessel tumor emboli and sentinel lymph node.The relationship between various factors and axillary lymph node metastasis were analyzed,and compared the value of sentinel lymph node biopsy and primary foci ADC values in the diagnosis of axillary lymph node metastasis.Results: The ADC values between axillary lymph node metastasis group and without axillary lymph node metastasis group had significant difference(P=0.013).Single factor analysis showed that the size of primary foci,ADC value,and the maximum diameter of axillary lymph node were related with axillary lymph node metastasis(P<0.05).Drawing the ROC curve according to the ADC values,the AUC aws 0.791(P=0.005,95%CI=0.630-0.952),when the threshold of ADC was set at 0.885×10-3 mm2/s,the diagnostic sensitivity and specificity were 84.2% and 71.4%,respectively.Conclusion: Primary foci DWI of breast cancer provides is a predictive value in the patients with axillary lymph node metastasis,and ADC values is an important diagnostic index.关键词
