误诊为脊柱关节炎的布鲁菌病11例临床分析

Abstract
Objective To investigate clinical characteristics of brucellosis with osteoarticular system lesions as main manifestation.Methods Clinical data of 11 patients with brucellosis, whose primary diagnosis was spinal arthritis from 2010 to 2016, was retrospectively analyzed.Results Among the 11 patients, the median age was 41 years old, and the median course of disease was 3 months.Among the 11 patients with osteoarticular system lesions, there were 8 patients with peripheral arthralgia, 5 patients with pain in gluteal region and 3 patients with lumbar pain.Nonspecific symptoms were found in 10 patients, and other systems lesions were found in 6 patients.The 11 patients were misdiagnosed as having spondyloarthropathy in primary diagnosis, and corresponding treatments such as dephlogisticate, analgesia and physiotherapeutics were given, but the effect was poor.History of direct or indirect contact with cattle and sheep was gotten after questioning insistently, and results of brucella agglutination test were positive in 11 patients, and then brucellosis was confirmed.Misdiagnosed time was 0.5-10 months.Combined anti-infective therapy was given after confirming the diagnosis with good prognosis.Conclusion Brucellosis with osteoarticular lesions as main symptom is common, and it is easily misdiagnosed as spondyloarthropathy, and therefore improving understanding of brucellosis, asking detailedly history especially the epidemiological history, paying attention to nonspecific symptoms and other systemic damages and performing special examinations early such as brucella agglutination test are helpful to confirm the diagnosis.关键词
