乌梅降糖止泻方佐治上热下寒型糖尿病伴顽固腹泻患者的效果及对胃肠动力指标的影响
Effectiveness of Smoked Plum Hypoglycemic Antidiarrheal and Effect on Gastrointestinal Motility Index in Adjunctive Therapy of Patients with Heat-syndrome in the Upper Part but Cold in the Lower Diabetes Complicated by Intractable Diarrhea

Abstract
Objective To observe effectiveness of smoked plum hypoglycemic antidiarrheal and effect on gastrointestinal function index in adjunctive therapy of patients with heat-syndrome in the upper part but cold in the lower diabetes mellitus complicated by intractable diarrhea.Methods A total of 271 patients with heat-syndrome in the upper part but cold in the lower diabetes mellitus complicated by intractable diarrhea admitted during October 2014 and December 2016 were randomly divided into blank control group(n=91), drug control group (n=90) and observation group (n=90) by random parallel control method.All patients received basic treatment of strict control of blood sugar.Blank control group was not treated with other treatments, and drug control group was treated with Smectite Powder, and observation group was given smoked plum hypoglycemic antidiarrheal treatment.The course of treatment was 1 month.After treatment, gastrointestinal symptoms scores, clinical effectiveness, blood glucose level and changes of gastrointestinal motility index before and after treatment and medication safety were evaluated in the 3 groups.Results After treatment, in observation group, 6 kinds of gastrointestinal symptom scores were significantly improved compared with those before treatment, and each gastrointestinal symptom score was significantly improved compared with those after treatment in black and drug control groups (P<0.05).The total effective rate in observation group was 90.00%, and the rate was significantly higher than 32.97% in blank control group and 73.33% in drug control group (P<0.05).Values of fasting blood glucose (FBG), 2 h postprandial blood glucose (2 h PBG) and glycosylated hemoglobin (HbA1c) levels after treatment were significantly better than those before treatment among the 3 groups (P<0.05), and the above indexes after treatment in observation group were better than those in blank and drug control groups (P<0.05).After treatment, orocecal transit time (OCTT), somatostatin (SS), motilin (MTL) and gastrin (GAS) were significant differences compared with those before treatment (P<0.05).In observation group, values of gastrointestinal dynamics indexes were better than those in blank and drug control groups (P<0.05).There was no obvious adverse drug reaction during treatment in observation group.Conclusion Smoked plum hypoglycemic antidiarrheal in adjunctive therapy of patients with heat-syndrome in the upper part but cold in the lower diabetes mellitus complicated by intractable diarrhea can significantly improve diarrhea, multiple gastrointestinal symptoms and function, reduce basic blood glucose with few adverse reaction and good patients' dependence.关键词
