Abstract
Objective To examine the prenatal ultrasonographic appearance and prognosis of fetal intra-abdominal umbilical vein anomalies.Methods Sixty-nine patients with abnormal fetal intra-abdominal umbilical vein determined by prenatal ultrasound examination in our hospital from January to December,2014 were retrospectively analyzed.Follow-up was arranged to evaluate the prognosis.Results Among the 69 patients,58 had persistent right umbilical vein,where the umbilical vein turned left after entering the liver and was found at the right side of the gallbladder on ultrasound.Five patients had umbilical vein varix,which was the widening of the fetal intra-abdominal umbilical vein,including 1 patient with diffuse dilatation and 4 patients with local ectasia.Another 5 patients were confirmed with missing vein duct accompanied by abnormal connection of the umbilical vein,where the umbilical vein was connected to the right atrium or inferior vena cava after entering the abdominal cavity.There was 1 patient with a fistula between the hepatic vein and the umbilical vein,which presented as the presence of the vein duct and the connection between the hepatic vein and the torturous umbilical vein.Out of the 69 patients,54 had no other deformities,including 3 cases lost to follow-up and 51 cases with good prognosis.Three patients had other mild deformities with good prognosis,including one with a single umbilical artery and two with ventricular septal defect.On the other hand,12 patients were found to have serious deformities (one case of trisomy 18 and two cases of trisomy 21) mainly in the cardiovascular,urinary,and nervous systems or facial regions,and thus labor was induced in all 12 patients.Conclusion Fetal intra-abdominal umbilical vein anomalies have characteristic ultrasonographic appearance.Patients with no other deformities have favorable prognosis,while those with serious deformities have poor prognosis.关键词
