Abstract
Objective To explore the effects of paraspinal approach one-stage decompression and reconstruction in the treatment of thoracolumbar fracture and dislocation.Metods A total of 60 patients with thoracolumbar fracture and dislocation were selected and randomly divided into an observation group and a control group (n =30,respectively).The observation group was treated by paraspinal approach and the control group by posterior median approach.The operation conditions,Cobb angle and height of anterior and posterior vertebral margin one month before and after the operation and the Prolo scores three and six months before and after the operation between the two groups were compared to evaluate the effects of paraspinal approach one-stage decompression and reconstruction in the treatment of thoracolumbar fracture and dislocation.Results The operation time and length of stay (LOS) in the observation group were shorter than those in the control group,and the amount of bleeding during operation and postoperative drainage were less than those in the control group (P ＜ 0.05).The height ratio of anterior posterior margin and Cobb angles between the two groups before the operation had no statistical difference(P ＞ 0.05).One month after the operation,the height of anterior margin in the two groups increased and that of posterior margin decreased,the value of Cobb value decreased (P ＜ 0.05),but there was no difference between the two groups (P ＞ 0.05).Three and six months after the operation,the Prolo scores in the two groups were higher than those before the operation,and the scores in the observation group were higher than those in the control group during the same period (P ＜ 0.05).Conclusion Paraspinal approach one-stage decompression and reconstruction has good effects in the treatment of thoracolumbar fracture and dislocation,can ensure effective restoration of the physiological curvature of spine,minor injury and fast recovery,and is beneficial to the recovery of patients' ability of daily living.关键词
