不典型主动脉夹层患者18例临床分析

Abstract
Objective To investigate the clinical features of atypical aortic dissection and to reduce misdiagnosis and missed diagnosis.Methods Retrospective analyzed of 18 cases of atypical aortic dissection from January 2010 to March 2017 at the emergency internal medicine of Xuanwu Hospital of Capital Medical University.The clinical manifestations and diagnosis and treatment were analyzed.Results There were 18 patients,16 males and 2 females,aged(52.2±10.2)years.17 cases were diagnosed by CT angiography in emergency department.In 16 patients had hypertension(88.9％),atypical symptoms of chest symptoms(6 cases),abdominal symptoms(5 cases),neurological symptoms(4 cases),respiratory symptoms(2 cases)and dysphagia(1 case).After surgery,4 cases were cured,5 cases were received conservative treatment,3 cases died,1 cases were sudden death,8 cases were transferred to other hospital.Conclusion The clinical manifestations of aortic dissection are diverse,and many patients have atypical symptoms.Emergency physicians should be vigilant to avoid misdiagnosis and missed diagnosis.关键词
