摘要
Abstract
Objective To explore the practice mode of pharmacists providing precision medication service,and to provide reference for carrying out pharmaceutical care.Methods By 1 case of medication reconciliation for a percutaneous coronary intervention (PCI) postoperative patient combined with his clinical manifestations,pharmacists provided advice on the medication therapy and made follow-up.Results and conclusion Pharmacists participating to the disease-associated medication therapy management can make the correct medication guide for patients and can guarantee the standardization and the rationality of the treatment.关键词
