左西孟坦注射液联合无创呼吸机治疗老年急性心力衰竭的临床研究
Clinical trial of levosimendan injection combined with noninvasive ventilator in the treatment of elderly patients with acute heart failure

Abstract
Objective To observe the clinical efficacy and safety of levosimendan injection combined with noninvasive ventilator in the treatment of elderly patients with acute heart failure.Methods One hundred and sixty-two elderly patients with acute heart failure were randomly divided into control group and treatment group with 81 cases per group.Two groups were received the noninvasive ventilator therapy.Control groups was treated with dobutamine hydrochloride injection 5 μg · kg-1 · min-1 with intravenous injection for 24 h.Treatment group was given levosimendan injection 12 μg · kg-1 with intravenous injection for 10 min,then intravenous injection for 24 h with 0.1 μg · kg-1 · min-1 The clinical efficacy,cardiac output (CO),cardiac index (CI) and heart rate (HR) and adverse drug reactions were compared between two groups.Results After treatment,the total effective rates in treatment and control groups were 96.29％ (78/81 cases) and 86.42％ (70/81 cases) with significant difference (P ＜ 0.05).After treatment,the main indexes in treatment and control groups were compared:the CO were (2.33 ±0.33) (1.79 ±0.24)L · min-1;the CI were (3.23 ±0.46),(2.35 ±0.32) L · min-1· m-2;HR were (97.65 ± 13.27),(112.12 ± 10.89) times · min-1,with statistically significant difference (all P ＜ 0.05).Adverse drug reactions in treatment group were based on dizziness,decreased blood pressure and flatulence,which in control group was based on dizziness and flatulence.The incidence of adverse drug reactions in treatment and control groups were 13.58％ and 11.11％,without significant difference (P ＞ 0.05).Conclusion Levosimendan injection combined with noninvasive ventilator has a definitive clinical efficacy in the treatment of elderly patients with acute heart failure,which can significantly reduce the HR,improve the levels of CO and CI,without increasing the incidence of adverse drug reactions.关键词
