摘要

Abstract

Objective To explore the the mechanism of tumor necrosis factor-α (TNF-α),etythropoietin (EPO) and interferon-γ(IFN-γ)on cancer relate anemia(CRA).Methods A total of 97 cases of malignant tumor patients were divided into treatment group (tumor with anemia,76 cases) and control group (tumor without anemia,21 cases);76 cases of CRA paitents accorded to the parameters of iron metabolism were divided into treatment Ⅰ group (29 cases) and treatment Ⅱ group (47 cases),another 50 healthy volunteers were selected as the normal group.The serum erythropoietin (EPO),ferritin (SF) level were detected by radioimmunoassay method.The serum TNF-α,IFN-γ level were detected by enzyme-linked immune adsorption method.Extracted of normal bone marrow,conventional cultured,added different concentrations of TNF-α,IFN-γ,red blood cell colony forming units (CFU-E) in bone marrow were counted,added different concentration of recombinant human erythropoietin (rhEPO) in culture system,the concentration of CFU-E were counted.Results The serum levels of TNF-α in treatment Ⅰ group,treatment Ⅱ group and control group were (137.8 ± 13.2),(241.4 ± 19.8),(113.7 ± 12.6) ng· L-1;levels of EPO in that three groups were(513.7 ±37.1),(268.8 ±21.9),(94.6 ± 11.4) ng· L-1;levels of IFN-γ in that three groups were (191.5 ± 16.5),(289.7 ± 20.3),(157.9 ± 14.7)ng ± L-1.The three factors were significantly higher than that of normal group (51.3 ± 8.4),(56.8 ± 7.1),(57.4 ± 7.8) ng · L-1 with significantly (P < 0.05).The serum EPO in treatment Ⅱ group was significantly higher than that in control group,but was significantly lower than that in treatment Ⅰ group with significantly (P < 0.05).The serum levels of TNF-α and IFN-γ in the treatment Ⅱ group were significantly higher than those in the treatment Ⅰ group and control group with significantly (P < 0.05).In vitro cell culture results showed that TNF-e,IFN-γ with inhibition effect on normal bone marrow CFU-E,and this inhi-bitory effect with concentration dependent-manner with the increased of concentration.The inhibitory effect of CFU-E cells in bone marrow was stronger.By added rhEPO in the culture system,the inhibition effect of TNF-α,IFN-γ could be improved in 2 × 10-10 ~32 × 10-10 mol · mL-1 range,the higher the concentration of rhEPO,the more obvious the improvement effect on the inhibition of TNF-α,IFN-γ;but still unable to return to normal level.Conclusion The regulatory factor of TNF-α,INF-γ effect on EPO,cause the reaction of bone marrow erythroid to EPO relative reduction,maybe one of the mechanisms of cancer relate anemia.
