Abstract
Objective To study the related factors of multiple organ dysfunction syndrome (MODS) in severe preeclampsia patients and to provide reference for clinical practice.Methods Altogether 523 severe preeclampsia patients admitted in Hangzhou Yuhang District First People''s Hospital from December 2007 to December 2015 were selected.Sixty-five patients with MODS were assigned into observation group, and 458 cases without MODS were divided into control group.Statistical analysis was performed on clinical data of all severe preeclampsia patients including age, gestational week, gravidity, parity, gestational week at termination, maternal and neonatal outcomes, prenatal examination, scarred uterus, medical history and treatment measures.Results There were significant differences in proportion of patients terminating pregnancy at ≤27 weeks, receiving prenatal examination irregularly, with scarred uterus, kidney disease, and chronic hypertension between the observation group and the control group (t value was 4.432, 7.448, 16.453, 9.107 and 5.900, respectively, all P<0.05).Relevance was shown between severe preeclampsia patients with MODS and termination of pregnancy at gestational age ≤27 weeks, irregular prenatal examination, scarred uterus, renal disease and chronic hypertension (OR value was 1.924, 3.278, 2.296, 2.846 and 3.297, respectively, all P<0.05).Proportion of patients requiring special treatment (including mechanical ventilation, admittance in ICU, transfusion of blood products) was significantly lower among patients without MODS than patients with MODS (t=30.192, P<0.05).Incidences of perinatal death, premature infant, intracranial hemorrhage and neonatal asphyxia in severe preeclampsia patients without MODS were significantly lower than those in patients with MODS (t value was 13.084, 4.434, 8.859 and 4.692, respectively, all P<0.05).Organ injury severity score of patients with MODS with gestational age less than 32 weeks was significantly higher than that of patients with MODS with gestational age >32 weeks (t=3.900, P<0.05), and gestational week of pregnancy termination of patients with MODS with gestational week ≤32 weeks was less than that of patients with MODS with gestational age >32 weeks (t=-2.533,P<0.05).Conclusion Risk factors of severe pereclampsia with MODS are gestational week of termination of pregnancy ≤27 weeks, irregular prenatal examination, scarred uterus, kidney diseases and chronic hypertension.Prognosis of severe preeclampsia patients with MODS is worse than that of patients without MODS, and the earlier the onset of MODS, the worse the prognosis of patients with MODS.关键词
