摘要

Abstract

Objective To evaluate the effect of combined nutrition management on nutritional status,complications and short-term prognosis in patients with severe stroke.Methods Ninety patients with severe stroke were randomly divided into control group (48 cases) and observation group (42 cases).Based on the early enteral nutrition support,the combined nutrition management of the observation group was carried out by combination with doctors,dietitians,nurses and rehabilitative therapist,the nutrition management of the control group was operated by nurses according to nutritionist advice.The alteration of the nutritional index including hemoglobin (Hb),serum albumin (ALB) and prealbumin (PA),total cholesterol (TC),triglyceride (TG) levels and arm muscle circumference (AMC),the incidence of complications with pulmonary infection,stress ulcer and urinary tract infection,the changes of NIHSS score and Barthel index and nutritional support satisfaction survey in first days after admission and twenty-first days in two groups were assessed.Results On the twenty-first day of hospitalization,the nutritional indexes of the two groups were decreased,and the nutritional indexes of the control group has decreased more obviously,and the nutritional indexes of the observation group were significantly better than those of the control group (P<0.05).The incidence of hypoproteinemia (19.04%) in the observation group was significantly lower than that in the control group (39.6%,P<0.05).The incidence of infection (23.8%) in the observation group was significantly lower than that in the control group (P<0.05).There were significant differences on NIHSS score and Barthel index in the twenty-first day between the two groups (P<0.05).The satisfaction rate of the observation group was significantly higher than that of the control group (P<0.05).Conclusion Combined nutrition management including physicians,nurses,nutritionists and rehabilitation therapist contribute to support the nutrition of patients with severe stroke,improve the quality of life of patients with stroke.It is worthy to further promotion in clinic.
