Abstract
The author believes that the value system and practice model of Chinas heart prevention and rehabilitation mainly reflect the "12345" points.It's include:adhere to the public health and the interests of the patients as the center;doctor-patient active interaction;self management of chronic disease and health awareness,knowledge and practice;prevention health care service system 4S;drug prescription,exercise prescription,nutrition prescription,psychological prescription (including sleep management),quit alcohol limit prescription five prescription.关键词
