Abstract
Objective To explore the therapeutic effect of Sildenafil combined with inhaling nitric oxide in the treatment of persistent pulmonary hypertension of newborn (PPHN). Method 116 cases of PPHN who were diagnosed and treated in our hospital from January 2013 to December 2016 were enrolled in this study and they were randomly divided into combination treatment group and control group, 58 cases in each group. Control group newborn were treated by inhaling nitric oxide (iNO), and combination treatment group newborn were treated by iNO combined with Sildenafil. The results of blood gas analysis[arterial partial pressure of oxygen (PaO2), partial pressure of carbon dioxide (PaCO2), arterial oxygen saturation (SaO2), lactic acid level], the cardiac function [left ventricular ejection fraction (LVEF), fraction shortening (FS), ratio of peak E velocity and peak A velocity (E/A), pulmonary arterial systolic pressure (PASP)], the therapeutic effects, the levels of serum endothelin-1 (ET-1) and plasma brain natriuretic peptide (NT-proBNP) between the two groups were observed and compared. Result After treatment, PaCO2 and lactic acid levels of the two groups were lower than those before treatment (P < 0.05), PaO2 and SaO2 were higher than those before treatment (P < 0.05), PaCO2 and lactic acid levels of combination treatment group were lower than control group (P < 0.05), PaO2 and SaO2 were higher than control group (P < 0.05). After treatment, LVEF, RVEF, FS and E/A of the two groups were higher than those before treatment (P < 0.05), PASP was lower than that before treatment (P < 0.05); LVEF, RVEF, FS and E/A of combined treatment group were higher than control group (P < 0.05), PASP was lower than control group (P < 0.05). The total effective rate of combined treatment group was higher than that of control group (P < 0.05). After treatment, the levels of ET-1 and NT-proBNP of the two groups were lower than that before treatment (P < 0.05), and the levels of ET-1 and NT-proBNP in combined treatment group were lower than that of control group (P < 0.05). Conclusion Sildenafil combined with iNO can effectively improve the clinical parameters of PPHN, which is better than inhaled nitric oxide alone.关键词
