Effects of Xinshen Kangning capsule combined with Western medicine on renal function and renal tubular injury in diabetic nephropathy

Abstract
Objective To investigate the clinical effects of kidney-reinforcing and blood circulation-activating traditional Chinese medicine (TCM) Xinshen Kangning capsule combined with Western medicine in the treatment of early and clinical diabetic nephropathy and its influence on renal function and renal tubular injury.Methods Sixty patients with early and clinical diabetic nephropathy were selected and were randomly divided into the observation group and control group with 30 cases in each group.Patients in the control group were treated with the conventional Western medicine, and patients in the observation group received three Xinshen Kangning capsules per time, 3 times/d, on the basis of routine Western medicine treatment.The two groups were treated for 6 months, the clinical efficacy and the effective rate of TCM syndromes were evaluated, and the total effective rate was calculated.At 3 and 6 months after treatment, fasting venous blood were collected to detect the serum glycated hemoglobin by liquid chromatography, to detect the serum Cr by TBA-120FR automatic biochemical analyzer, and to calculate creatinine clearance rate (Ccr).We preserve the urine.The urinary albumin, 24 h urinary protein quantitation and the renal tubular injury specificity urinary N-acetyl-β-D-glucosaminidase (NAG) and urine β2 microglobulin (β2-MG) were determined by immune turbidimetry.Results At 6 month after treatment, the clinical efficacy and the total effective rate of TCM syndromes in the observation group were higher than those in the control group (both P<0.05).At 3 and 6 months, the serum glycosylated hemoglobin, serum Cr, urinary microalbumin, 24 h urine protein quantitation, urine NAG, and β2-MG in the two groups were lower than those before treatment, and Ccr was higher than that before treatment (all P<0.05).At 6 month after treatment, the serum glycosylated hemoglobin, serum Cr, urinary microalbumin, 24 h urine protein quantitation, urine NAG and β2-MG in the observation group were lower than those in the control group, and Ccr was higher than that in the control group (all P<0.05).Conclusion The kidney-reinforcing and blood circulation-activating TCM Xinshen Kangning capsule combined with Western medicine can significantly improve the clinical symptoms and TCM syndromes in the early stage and clinical stage of diabetic nephropathy, improve renal function, reduce the renal tubular injury, and promote the recovery of renal function.关键词
