重症急性胰腺炎伴代谢综合征者手术治疗的疗效分析

Abstract
Objective To analyze the curative effect of surgery for severe acute pancreatitis complicated with metabolic syndrome.Methods The clinical data of 400 severe acute pancreatitis cases admitted to Shanghai Putuo District Central Hospital from 2010 to 2016 were retrospectively analyzed,including 162 severe acute pancreatitis complicated with metabolic syndrome(MS).The cure rate,complication rate and death rate were recorded and compared after they took surgeries or non-surgical treatment.Results The cure rate was higher in the severe acute pancreatitis patients treated by surgery than that of non-surgery treatment(g3.0％ vs 73.5％).The death rate in the severe acute pancreatitis patients treated by non-surgery was higher than that of surgery treatment (26.5％ vs 17.0％).The incidence of complication in the severe acute pancreatitis cases treated by non-surgery treatment was higher than that of patients treated by surgery(24.0％ vs 18.0％).The three indicators were statistically different between three groups(P ＜0.05).Among the severe acute pancreatitis patients complicated with MC,the cure rate of the patients treated by surgery was higher than that of patients treated by non-surgical treatment(75.0％ vs 60.0％,P ＜ 0.05),the death rate of the patients treated by non-surgery treatment was higher than that of patients treated by surgery(40.0％ vs 25.0％,P ＜0.05),and the incidence of complication of the patients treated by non-surgery treatment was higher than that of patients treated by surgery (41.1％ vs 27.8％,P ＜ 0.05).Among the severe acute pancreatitis patients complicated without MC,the cure rate,death rate and incidence of complications showed no significant difference between non-surgical treatment group and surgery group (P ＞ 0.05);Conclusion The surgical indication for cases of severe acute pancreatitis complicated with metabolic syndrome should be more flexible,and the timely surgery may increase the cure rate.关键词
