Abstract

Objective To investigate the application value of high-risk human pappilomavirus (HR-HPV) testing in the screening of cervical lesion.Methods A total of 287 women with positive outcomes of thinprep cytologic test (TCT) were subject to detection of HPV type.Among them, 168 cases received colposcopy combined with biopsy.The incidence of HPV infection among patients with different TCT and pathological staging was analyzed.Results Of 287 cases with positive TCT outcomes, the positive rate of HR-HPV in those with atypical squamous cell (ASC), low-grade squamous intraepithelial lesion (LSIL) and high-grade squamous intraepithelial lesion (HSIL) was 32%, 78% and 95%, respectively.Among 168 women undergoing colposcopy, 43 cases were pathologically diagnosed with normal or inflammation, 108 with cervical intraepithelial neoplasias Ⅰ (CIN Ⅰ) and 17 with CIN Ⅱ-Ⅲ, and the positive rate of HR-HPV was 2%, 60% and 94%, respectively.The higher grade by the cytological and pathological diagnosis, the higher positive rate of HR-HPV (both P<0.01).Of 82 cases with positive HPV, 99% of them were pathologically diagnosed with CIN I or above.Among 102 ASC women diagnosed by TCT and colposcopy, 42 cases presented with no pathological changes including 41 without HR-HPV infection.Conclusions Detection of HR-HPV can effectively make a diagnosis of cervical lesion.Detection of HR-HPV combined with TCT can reduce the risk of over-diagnosis and over-treatment.
