温针灸疗配合外敷自制威灵仙浸膏治疗膝骨性关节炎临床研究
Clinical Study of KOA with Needle-warming Moxibustion and External Application of Self-made Radix Clematidis Extract

Abstract
Objective:To observe the effect of needle-warming moxibustion combined with self-made Radix Clematidis (Weilingxian) extract for knee osteoarthritis(KOA),to study its action mechanism,and to provide comprehensive therapy for KOA.Methods:244 cases were randomly divided into the electro-acupuncture group(group E),the needle-warming moxibustion group(group M),the comprehensive A group (group A),and the comprehensive B group(group B),with 61 cases in each group.The four groups were all treated with Glucosamine Hydrochloride capsules;Points of ST34 (Liangqiu),EX-LE5 (Xiyan),GB34 (Yanglingquan),ST36(Zusanli),SP10(Xuehai) were selected,which were with needle-warming moxibustion in group M and with electro-acupuncture in group E;whereas external applications of Radix Clematidis extract were used both after needle-warming moxibustion in group B and after electro-acupuncture in group A.All intervention duration was two weeks.Results:After two weeks of treatment,X-ray examination results were all significantly improved in the four groups,in which group A and group B were superior to group M and group E,but there was no significant difference between group A and group B;the positive expression of MMP-1B (matrix metalloproteinase-1),the decrease of IL-1β (interleukin-1β),and the increase of HA (hyaluronic acid) in mesochondrium of knee joints in group B were more significant compared to those in the other three groups (P ＜0.05);in which,group A was superior to group M and group E with a statistical significance(P ＜0.05).The scores of VAS(visual analogue scale) and WOMAC (Western Ontario and McMaster Universities osteoarthritis index),recovery efficiency,and the total effective rate in group B were statistically superior to those in group A,group M and group E (P ＜ 0.05).Conclusion:Needle-warming moxibustion combined with self-made Radix Clematidis extract is a reliable comprehensive treatment for KOA.关键词
