低分子肝素辅助治疗早发型重度子痫前期的临床效果
Clinical effect of adjuvant therapy with low-molecular heparin on early-onset severe pre-eclampsia

Abstract
Objective To observe the clinical effect of adjuvant therapy with low-molecular heparin on early-onset severe pre-eclampsia(EOSP).Methods Sixty-three pregnant women with EOSP were divided into control group(n=29) and study group(n=34).The control group was given the conventional treatment while the study group was given low-molecular heparin on the basis of conventional treatment.The hematocrit(HCT),PLT,fibrinogen(FIB),D-dimer,activated partial thromboplastin time(APTT),thrombin time(TT),postpartum blood loss,and fetal umbilical artery ratio of end-systolic peak to end-diastolic peak(S/D),gestational age delay,neonatal Apgar score,time for terminating pregnancy and incidence of stillbirth were compared between the two groups before and after treatment.Results (1) After treatment,HCT,PLT,FIB and D-dimer were lower in the study group compared to the pre-treatment(all P>0.05),APTT was longer and the levels of FIB and D-dimer were lower in the study group compared to the control group(all P<0.05) (2) There was no statistically significant difference between two groups in the postpartum blood loss or neonatal weight(all P>0.05).The rate of terminating pregnancy within 3 or 5 days of hospitalization,S/D ratio and incidence rate of stillbirth were lower,gestational age delay and gestational age for terminating the pregnancy were longer,and neonatal 1-minute Apgar score was higher in the study group compared to the control group(all P<0.05).Conclusion For pregnant women with EOSP,low-molecular heparin can alleviate the high coagulation state and improve microcirculation,thereby extend the gestational age,improve the perinatal outcomes,and reduce the maternal and fetal complications.It is an effective and safe approach which does not increase blood loss.关键词
