Abstract
Objective To evaluate the efficacy and safety of intravenous and topical tranexamic acid(TXA) on blood loss in total knee arthroplasty.Methods The 25 patients who did not receive TXA were controls,whereas the other 25 patients who received intravenous and topical TXA randomly constituted the treatment group.Comparison was made between both groups with regard to intra-operative blood loss,postoperative drainage volume,postoperative reduction in haemoglobin and haematocrit levels,blood transfusion rate,incidence rate of deep vein thrombosis or pulmonary embolism.Results Intra-operative blood loss: treatment group (50.21±15.37)ml and control group (70.50±11.15)ml (P<0.05);postoperative drainage volume at d1,d2 postoperation: treatment group (105.34±34.98)ml and(45.25±15.29)ml,control group(119.56±36.46)ml and(70.54±20.07)ml(P<0.05);postoperative reduction in haemoglobin and haematocrit levels: d1 treatment group(23.11±7.45) g/L and 7.69%±2.16%,control group (31.15±12.21) g/L and 9.87%±4.75% (P<0.05);d3 treatment group (29.73±10.78) g/L and 8.13%±3.22%,control group(39.64±8.77) g/L and 10.46%±4.01% (P<0.05);d6 treatment group(30.58±14.11) g/L and 9.69%±3.95%,control group(40.16±11.10) g/L and 11.01%±3.15%(P<0.05);d9 treatment group(19.85±10.84) g/L and 4.20%±2.11%,control group(30.05±9.68) g/L and 4.57%±2.01% (P<0.05);3 patients in the control group received blood transfusion(P<0.05).No patient was diagnosed deep vein thrombosis or pulmonary embolism.Conclusions 1 g intravenous TXA given 10 minutes prior to TKA and 1 g topical TXA given before closing incision is an effective and safe means of decreasing blood loss and reduction in haemoglobin concentrations,without increasing the risk of thromboembolic events.关键词
