非典型症状糖尿病误诊七例临床分析

Abstract
Objective To investigate clinical characteristics, misdiagnosed causes and preventative measures of atypical diabetes mellitus (DM).Methods Clinical data of 7 patients with atypical DM, in whom 54 misdiagnosed patients had admitted during May 2015 and May 2016, was retrospectively analyzed.Results The misdiagnosis rate was 12.96%, and primary manifestations of the 7 patients were symptoms of DM complications, and then the primary diagnoses were skin ulcer in 1 patient, right lower extremity osteomyelitis in 1 patient, acute upper respiratory tract infection in 1 patient, cerebral arteriosclerosis in 1 patient, acute gastroenteritis in 1 patient, acute cholecystitis in 1 patient and eczema in 1 patient.Misdiagnosed time was 1-9 d.All the 7 patients were confirmed diagnosis by tests such as blood glucose test, and there were 2 patients with typeⅠ DM and 5 patient with typeⅡ DM.All patients received oral medication or insulin hypoglycemic treatment and symptomatic treatments for complications, and patients were discharged when patients were cured or conditions were obviously improved.Conclusion Symptoms of atypical DM are lack of typical features of DM, and therefore it is easily misdiagnosed because of decreased range of clinicians.Clinicians should strengthen the understanding of atypical symptoms of DM and enhance vigilance of the disease in order to avoid misdiagnosis.关键词
