Abstract
Objective To investigate the correlation of prolonged QRS wave duration with serum NT-proBNP and recent prognosis in hypertensive elderly petients with heart failure.Methods 177 cases of hypertensive elderly petients with heart failure were selected as research subject from the Affiliated Hospital of School of Medicine of Ningbo University from February 2015 to April 2016.The cases were divide into MACE group (n=55) and non-MACE group (n=122) according to major adverse cardiac events (MACE) with follow-up of 6 months after discharge;and the cases were also divide into QRS wave duration≤120 ms group(n=113) and QRS wave duration>120 ms group (n=64).General clinical data and laboratory examinations were collected on admission and compared the differences between the two groups.Correlation of QRS wave duration with NT-proBNP and related cardiac function indexes by using Pearson correlation analysis;and the relationship of QRS wave duration between recent prognosis by using Logistic regression equation analysis.Results Compared with non-MACE group,left ventricular ejection fraction (LVEF),Systolic blood pressure(SBP),diastolic blood pressure(DBP),left ventricular mass index (LVMI),fasting plasma glucose (FPG),serum uric acid,NT-proBNP and QRS wave duration of MACE group were higher,the differences were statistically significant(P<0.05).Compared with QRS wave duration≤120 ms group,LVEF was lower,NT-proBNP,LVMI and MACE incidence were higher,the differences were statistically significant(P<0.05).Pearson correlation analysis showed that QRS wave duration was positively related with the NT-proBNP (r=0.352,P<0.05);Logistic regression equation analysis showed that prolonged QRS wave duration was independent risk factors of MACE in hypertensive elderly petients with heart failure〔OR=1.942,95%CI(1.260,2.839)〕.Conclusion Prolonged QRS wave duration was related with serum NT-proBNP and poor prognosis in these patiens,and may be an independent predictor of poor prognosis and should be pay more clinical attention.关键词
