Abstract
Objective To investigate the effect of etoricoxib combination therapy on knee osteoarthritis and its effect on inflammatory mediators.Methods A total of 144 patients with osteoarthritis of the knee were selected from February 2015 to September 2016 in Zhejiang University of Traditional Chinese Medicine Affiliated Wenzhou Chinese Medicine Hospital.They were randomly divided into four groups:the etoricoxib group,the functional training group,the combined group and the health education group.The WOMAC scores,the 6-min walking distance and the SF-36 score of all patients before and after treatment were compared between the four groups of patients.The levels of serum NF-κb,IL-1β and MMP-1 in the four groups were compared with those of the four groups before and after treatment.The adverse effects were observed in each group.Results There was no significant difference in the WOMAC score between the groups before treatment(P>0.05).The WOMAC scores of the functional training group,the combined group and the health education group were lower than the groups before treatment(P<0.05).There was no significant difference in the WOMAC score of the etoricoxib group between before and after treatment(P>0.05).The WOMAC scores of the combined group after treatment were lower than the WOMAC score of the etoricoxib group,the functional training group and the health education group(P<0.05).There was no significant difference between the 6-min walking distance and the SF-36 score in the four groups before treatment(P>0.05).The 6-min walking distance and SF-36 score of the etoricoxib group,the functional training group and the combined group after treatment were higher than before treatment(P<0.05).The 6-min walking distance and SF-36 score of the combined group were higher than those of the etoricoxib group,the functional training group and the health education group(P<0.05).There was no significant difference in the levels of NF-κb,IL-1β and MMP-1 between the four groups before treatment(P>0.05).The levels of NF-κb,IL-1β and MMP-1 were significantly lower in the etoricoxib group,the functional training group and the combination group than before treatment(P<0.05).The levels of NF-κb,IL-1β and MMP-1 in the combined group were lower than those in the etoricoxib group,the functional training group and the health education group(P<0.05).Among them,there were mild symptoms of gastrointestinal reaction in 3 cases of the etoricoxib group and the combined group,and the incidence of adverse reactions in each group was not statistically significant(P>0.05).Conclusion Based on the combination of etoricoxiband functional training training,which can significantly improve the KOA patient′s knee function,reduce the expression of serum inflammatory regulatory factors,and the clinical application is safe.关键词
