Abstract
Objective To evaluate the relationship between the level of plasma homocysteine and urinary microalbumin(MAU) in different glucose tolerance groups.Methods We enrolled patients in the Community Health Service Center of Beijing University of Posts and Telecommunications from March 2014 to June 2016,the subjects were divided into 5 groups according to different levels of glucose metabolism and MAU:NGT group(n=240),IGR group(n=230),IGR+MAU group(n=31),T2DM group(n=180),T2DM+MAU group(n=55).The general situation of each group was recorded,including sex,age and course of disease;the body height,body mass,systolic and diastolic blood pressure were recorded,and body mass index(BMI) was calculated;serum biochemical indexes,Hcy levels and ACR were measured.Spearman correlation analysis and multiple Logistic regression were conducted to analyze influencing factors of MAU.Results There was no significant difference in diastolic blood pressure,total cholesterol(TC) and high-density lipoprotein cholesterol(HDL-C) among the 5 groups(P>0.05);there was significant difference in BMI,systolic blood pressure,fasting blood glucose(FPG),glycosylated hemoglobin(HbA1c),three triacylglycerol(TG),low-density lipoprotein cholesterol(LDL-C),Hcy,ACR among the 5 groups(P<0.05).Systolic blood pressure,FPG,HbA1c,Hcy and ACR in IGR+MAU group were higher than those in NGT group and IGR group,the difference was statistically significant(P<0.05);the BMI in T2DM group was higher than that in NGT group,the difference was statistically significant(P<0.05);the BMI,TG,LDL-C level in T2DM+MAU group were higher than those in NGT group,systolic blood pressure,FPG,HbA1c,Hcy,ACR in T2DM+MAU group were lower than those in NGT group and IGR group,the differences were statistically significant(P<0.05).Pearson correlation analysis showed that MAU and BMI,systolic blood pressure,HbA1c and Hcy were positively correlated in IGR+MAU group(r=0.48,r=0.47,r=0.51,r=0.77,P<0.05).Multivariate Logistic regression analysis showed that BMI,course of disease,systolic blood pressure,FPG,HbA1c and Hcy were independent risk factors of MAU(P<0.05).Conclusion BMI,duration of disease,systolic blood pressure,FPG,HbA1c and Hcy are independent risk factors of MAU.关键词
