Abstract
Objective:To investigate the clinical value of serum cystatin C (CysC),retinol binding protein (RBP),and urinary transferrin (TRF) and microalbumin (mAlb) levels in early diagnosis of diabetie nephropathy (DN).Methods:74 patients with type 2 diabetes treated in our hospital from January 2014 to January 2016 were selected and divided into 3 groups according to different urinary albumin excretion rates (UAER):diabetic group (n=24,UAER＜30 mg/d),early DN group (n=25,UAER 30～300 mg/d) and DN group (n=25,UAER＞300 mg/d).36 healthy volunteers were enrolled as the control group.The levels of CysC and RBP in serum,and urinary TRF and mAlb were mearured,and the value of these indexes for DN was analyzed.Results:The levels and positive rates of CysC,RBP,TRF and mAlb in early DN and DN groups were higher than those in diabetic group and control group (P＜0.05),and were the highest in DN group (P＜0.05).The levels of CysC,RBP,TRF and mAlb in patients with diabetes or DN were negatively correlated with glomerular filtration rate (GFR) (P＜0.05),whereas no significant association was observed between any of these indexes and GFR in the controls (P＞0.05).The serum levels of CysC and RBP as well as urinary TRF and mAlb possessed good diagnostic performances for DN.The sensitivity of this combined detection was 68.0％,and the specificity was 100.0％.Conclusion:Detecting the levels of serum CysC and RBP,and urinary TRF and mAlb was of important significance in the early diagnosis of DN.关键词
