中西医结合治疗老年消化性溃疡临床研究
Clinical Study on Treatment of Senile Peptic Ulcer with Intetgrative Medicines

Abstract
Objective:To observe the clinical curative effect of Jianpi Changluo Decoction in treatment of senile peptic ulcer (with Spleen Deficiency and Coldness Syndrome and Blood Stasis Syndrome) and the effect of it on patients' spleen gastrin (GAS),epidermal growth factor (EGF)),Thromboxane B2 (TXB2) and 6-keto prostaglandin fla (6-Keto-PGFla) levels.Methods:104 elderly patients with peptic ulcer were randomly divided into control group and observation group,with 52 cases in each group.The control group was treated with 30 mg lansoprazole,once a day and 4 bags Livzon dele,four times,one bag each time.If HP is positive,amoxicillin capsules and clarithromycin were added in treatment.The observation group was treated with Jianpi Huoluo Decoction in addition to that of the control group.Both groups were treated for 8 weeks.The TCM score,gastroscopy and Hp before and after treatment were observed.Before and after treatment,endoscopy and Hp detection was made The levels of serum GAS,EGF,TXB2 and 6-Keto-PGFla were measured before and after treatment,and the safety evaluation was carried out before and after treatment.Results:Compared with that (76.92％) of the control group,the effective rate of the observation group was 92.31％ (X2 =4.727,P ＜ 0.05).The effective rate of the observation group was 88.46％,which was significantly higher than that (73.08 ％) of the control group (X2 =3.961,P ＜ 0.05).After treatment,the symptoms scores on epigastric pain,upper abdominal fullness,mental fatigue,less appetite,black melena and vomiting of clear water,etc.of the observation group were statistically lower than that of the control group (P ＜ 0.01).The HP negative conversion rate was 94.28％ in the observation group and it was higher than that (78.37％) in the control group,but the difference was not statistically significant (P ＞ 0.05).After treatment,the GAS and 6-Keto-PGFla of the observation group was significantly lower than that of the control group,while the increase of TXB2 and EGF was significantly higher than that of the control group (P ＜ 0.01).There were no serious adverse events in the two groups,and no adverse reactions related with TCM in the two groups.Conclusion:Integrative treatment of senile peptic ulcer can further promote the healing of ulcer,improve the clinical symptoms,the clinical efficacy and safety,and its mechanism may be related to the regulation of GAS,EGF,TXB2 and 6-Keto-PGFla levels.关键词
