非免疫性胎儿水肿临床分析
Clinical analysis of nonimmune hydrops fetals
摘要
Abstract
Objective:To investigate the clinical characteristics,treatment and prognosis of nonimmune hydrops fetalis (NIHF).Methods:To retrospectively analyzed 20 cases of hydrops fetalis which were treated in the First Affiliated Hospital of Nanjing Medical University from July 2010 to June 2015.The details of fetus and pregnancy women included clinical characteristics,treatment and prognosis were explored.Results:20 cases of hydrops fetalis are nonimmune hydrops fetalis (NIHF).It was found that diagnosis time of hydrops fetalis from pregnancy 14+ to 37+ weeks,2 cases in early pregnancy,4 cases in mid-pregnancy and 14 cases in late pregnancy.There were 17 cases with pleural effusion,9 cases with ascites,11 cases with skin edema and 9 cases with hydramnion.3 cases of absent end-diastolic flow of umbilical artery and 4 cases of fetal abnormalities.12 cases with hydrops fetalis syndrome,among them 7 cases of patients choose to aborted and induced labor,the other outcome were fetal death,stillbirth and neonatal death.There were 8 cases with atypical hydrops fetalis,the outcome of 2 cases were neonatal death,4 cases have good prognosis and 2 cases lost to follow-up.Conclusion:It was remains unclear for the pathogenesis of nonimmune hydrops fetalis (NIHF),while its clinical manifestations are complex and diverse.The prognosis of hydrops fetalis syndrome was worse.According to different condition,we should implement careful monitoring,individual therapy and choose adaptable delivery.关键词
