Abstract
Objective:To investigate the effect of different type of laparoscopic gastrectomy reconstruction on blood sugar of patients with gastric cancer combined with type 2 diabetes.Methods:Sixty-six patients of gastric cancer combined with types 2 diabetes who underwent surgical treatment were collected and divided into BillrothⅠgroup (26 cases),Billroth Ⅱ group (24 cases) and Roux-en-Y group (16 cases).The oral glucose tolerance test (OGTT) fasting,challenged blood glucose levels and glycosylated hemoglobin were collected and compared among the 3 groups before and after follow-up of 3 months.Results:Before surgery,the glycosylated hemoglobin levels,challenged glucose after 2 h,OGTT fasting glucose,fasting insulin levels and the homeostasis model assessment of insulin resistance (HOMA-IR) of the three groups were similar (P>0.05).After three-months' follow-up,the rate of diabetes remission of Billroth Ⅱ group and Roux-en-Y group were 66.67% and 87.50% respectively,which were both higher than that of the BillrothⅠgroup (19.23%,P<0.05),while the OGTT fasting glucose,OGTT 2 h glucose value (P=0.040),glycosylated hemoglobin,fasting insulin and HOMA-IR were significantly lower than those of the BillrothⅠgroup (P<0.05).Above indicators between the Roux-en-Y group and Billroth Ⅱ group had significant differences (P<0.05).Conclusions:BillrothⅡand Roux-en-Y gastrojejunostomy can decline the blood glucose effectively,and the Roux-en-Y gastric bypass is more effective than BillrothⅡ gastric bypass.关键词
