老年患者1例复杂用药引起多发药物不良反应的药学监护
Pharmaceutical care of multiple adverse drug reactions caused by complex medication prescription in an elderly patient
摘要
Abstract
Objective To explorate the pharmaceutical care in elderly patients with complex medication condition.Methods Clinical pharmacists helped in a case where multiple adverse drug reactions was caused by complex medication prescription to an elderly patient.Through drug reforming,improving treatment,pharmacy recommendations made,individualized pharmaceutical care was conducted.Results and conclusion According to the actual demand in the patient,the kinds of used drugs were down regulated to 13 from 30,and the patient's oral cavity ulceration,bone pain,muscle pain,such as adverse drug reaction was significantly improved,the overall better.Pharmaceutical care of elderly patients with complex medication and pharmaceutical service mode,can focus on three aspects:streamlining the drug use,improving adverse drug reactions,and increasing patient compliance.关键词
