非甾体类抗炎药对中轴型脊柱关节炎血清血管内皮生长因子和骨形态发生蛋白-2影响研究
Influence of non-steroidal anti-inflammatory drugs on serum VEGF and BMP-2 levels in patients with axial spondyloarthritis

Abstract
Objective To investigate the serum levels of VEGF and BMP-2 in patients receiving axial spondyloarthritis treatment with non-steroidal anti-inflammatory drug and the possible relationship with disease activity.Methods Totally 120 patients with axial spondyloarthritis were recruited for this study at the First Affiliated Hospital of Zhengzhou University from October 2014 to September 2015.They were randomized into two groups,imrecoxib group and celecoxib group,60 in each group,After 3 months of treatment,the levels of serum VEGF and BMP-2 were detected by ELISA methods.The clinical parameters,which included laboratory parameters (ESR,CRP),disease activity (BASDAI score and patients global assessment of disease activity),functional evaluation (BASFI,tragus-up-wall distance,lumbar side flexion,intermalleoar distance,Schober test,finger to floor distance) and SPARCC score,were measured before and after treatment.Results A statistically significant difference was found in ESR,BASDAI,patients global assessment of disease activit,Schober test and SPARCC score before and after treatment(P<0.05).However,there was no statistically significant difference in BASFI,tragus-up-wall distance or lumbar side flexion (P> 0.05).There were no statistically significant difference between the two groups.(P> 0.05).There was statistically significant difference on serum VEGF levels before and after treatment [(240.89 ± 17.68) ng/L vs (187.00 ± 11.42) ng/L,P<0.05),however,not in BMP-2[(231.74 ± 104.44)ng/L vs (226.80 ± 116.26) ng/L,P> 0.05].A significant correlation was found between VEGF level and ESR,CRP,BASFI,tragus-up-wall distance and finger to floor distance,lumbar side flexion,Schober test and intermalleoar distance (r =0.628,0.542,0.238,0.299,0.353,-0.369,-0.373,-0.359,-0.274,P<0.05).The BMP-2 levels were correlated with CRP and lumbar side flexion (r=0.213,-0.190,P<0.05).Moreover,VEGF was positively correlated with serum levels of BMP-2 (r =0.326,P<0.05).Serum VEGF levels were significantly increased in HLA-B27-positive patients(P<0.05).Conclution Non-steroidal anti-inflammatory drugs not only can improve symptoms and function,but also can reduce sacroiliitis and delay radiographic progression possibly by affecting the levels of VEGF and BMP,and imrecoxib and celecoxib have a same efficacy.Serum VEGF may be considered as indicators of disease activity,functional status and response to treatment in axial SPA,and this effect is correlated with the expression of HLA-B27.关键词
