PDCA循环管理对规范Ⅰ类切口手术围术期预防性应用抗菌药物的效果分析
Analysis on Effect of PDCA Cycle Management in the Standardization of Prophylactic Application of Antibiotics in Perioperative Period of Type Ⅰ Incision Operations

Abstract
OBJECTIVE:To probe into the effect of plan-do-check-action ( PDCA ) cycle management in the standardization of prophylactic application of antibiotics in perioperative period of type Ⅰ incision operations in the Second Affiliated Hospital of Shantou University Medical College ( hereinafter referred to as "our hospital") . METHODS:1200 pieces of medical history were divided into before intervention ( Jan.-Jun. 2015 ) and after intervention( Jan.-Jun. 2016 ) according to the implementary time of PDCA cycle management, with 600 pieces in each. Of the each 600 cases, there were 221 pieces of prophylactic application of antibiotics before intervention, and 154 pieces after intervention, the effect of PDCA cycle management in the standardization of prophylactic application of antibiotics in perioperative period of typeⅠincision operations was analyzed. RESULTS:Before intervention, the rate of prophylactic application of antibiotics in type Ⅰ incision operations was 36. 83％ ( 221/600 ) , the rational rate of categories selection of prophylactic application of antibiotics was 29. 86％ ( 66/221 ) , the drug combination rate was 7. 24％ (16/221), the rational rate of drug administration time was 40. 27％ (89/221) and the 24 h withdraw rate was 11. 31％ (25/221), and the data after intervention were respectively 25. 67％ (154/600), 59. 09％ (91/154), 3. 25％ (5/154), 73. 38％ (113/154) and 38. 31％ (59/154), the differences were statistically significant (P<0. 05). CONCLUSIONS: After the implementation of PDCA cycle management, the rationality of prophylactic application of antibiotics in perioperative period of type Ⅰ incision operations in our hospital has been improved significantly, however, there are still irrational drug application needs further intervention.关键词
