关节镜手术联合中药治疗膝骨性关节炎疗效及机理
Effect of Arthroscopic Surgery combined with Duhuojisheng Decoction on the Expression of MMP-13 and IL-18 in Patients with Knee Osteoarthritis

Abstract
Objective To study the effect of arthroscopic surgery combined with Duhuojisheng Decoction on knee osteoarthritis (KOA) patients on the expression of interleukin-18 (IL-18) and matrix metalloproteinase 13 (MMP-13). Methods Totally 50 patients were randomly divided into treatment group underwent ar-throscopic surgery combined with oral Duhuojisheng Decoction, and the control group only underwent arthroscop-ic treatment. Before and after surgery, IL-18 and MMP-13 were examined in synovial fluid. Results In treatment group, the postoperative levels of IL-18 [(157.359 ± 97.923) pg/mL] and MMP-13[(113.692 ±100.549) μg/L] were higher than preoperative levels of IL-18[(237.254±94.528) pg/mL] and MMP-13[(176.692± 90.213) g/L] (P<0.05). Compared with control group, the level of MMP-13 in treatment group after operation was lower than that in the control group [(154.547±95.971) μg/L]. In control group, the level of IL-18[(176.203± 102.478) pg/mL] after operation was higher than that of preoperation [(251.546±91.937 pg/mL]. There was no sta-tistically significant between the two groups on the levels of IL-18 and MMP-13 after operation. In the treat-ment group, the VAS score(2.3 ± 1.6) and Lysholm score(54.7 ± 4.1) of postoperation were statistically different from the preoperative VAS score (7.2±1.8) and Lysholm score (89.3±2.9) (P<0.05). Conclusion Duhuojish-eng Decoction combined with arthroscopic surgery can inhibit the expression of IL-18 and MMP-13 in KOA pa-tients. It may have an effect on slowing the arthritis development process, relieving pain, improving the function of knee joint.关键词
