前庭神经炎五例临床误诊分析
Misdiagnosis Analysis of 5 Patients with Vestibular Neuritis

Abstract
Objective To summarize the features of vestibular neuritis, to explore the misdiagnosis causes of vestibu-lar neuritis and to improve the level of diagnosis and treatment. Methods The clinical data of 5 cases of vestibular neuritis treated in the Department of Otorhinolaryngology Head and Neck Surgery, Navy General Hospital from October 2016 and Feb-ruary 2017 were retrospectively analyzed. Results The average age of patients was 41. 4 years, and all patients presented with acute spontaneous vertigo with nausea/vomiting and unsteadiness. All 5 cases were initially misdiagnosed. Four cases were misdiagnosed as benign paroxysmal positioning vertigo and one was misdiagnosed as Meniere disease. All 5 cases were trans-ferred to our hospital, which showed spontaneous torsional nystagmus beating away from the lesion side, oscillopsia and unilat-eral vestibular dysfunction or vestibular hypofunction with the positioning test, video head pulse test and vestibular caloric test, and diagnosed as vestibular neuritis. All vestibular neuritis patients underwent vestibular rehabilitation and immunity enhance-ment. Treatment time in 4 cases was 1-4 weeks, 4 cases of vertigo disappeared and 1 case was lost in the follow-up. Conclu-sion Vestibular neuritis is a common disorder of vestibule, which is characterized by acute spontaneous vertigo, but it tends to be misdiagnosed for the lack of specificity. Clinicians must strengthen the understanding of the physiological and pathologi-cal features of vertigo. For patients with acute vertigo, detailed history, careful physical examination and differential diagno-sis, targeted for related medical examinations should be taken in order to avoid misdiagnosis. .关键词
