人工耳蜗植入术中脑脊液井喷的原因分析及处理
Causes and Management of Intra-operative Cerebrospinal Fluid Gusher and Post-operative Cerebrospinal Fluid Otorrhea/rhinorrhea in Cochlear Implantation

Abstract
Objective To report cerebrospinal fluid (CSF) gusher encountered in cochlear implantation (CI), in-cluding its etiology, management strategy and outcomes, in order to provide basis for its reduction. Methods CSF gush-er cases (n=40) in our department from Jan. 2000 to Dec. 2015 were retrospectively reviewed, including imaging find-ings, intra-operative management and outcomes. Results The rate of gusher during operation was 1.64% (40/2443) among our patients who underwent CI, of whom 3 experienced post-operative CSF otorrhea/rhinorrhea, with meningitis requiring surgical intervention in 2 patients and extraction of the cochlear implant in 1 patient. Outcomes were good in all cases with CSF gusher, which demonstrated various kinds of inner ear malformation. Conclusions CSF gusher can occur in CI patients with inner ear malformations, although with a low occurrence of post-operational complications in-cluding CSF otorrhea/rhinorrhea and meningitis. Outcomes are generally good with proper management.关键词
