中医综合疗法治疗慢性肩关节周围炎临床研究
Clinical Research of Integrated Traditional Chinese Medicine Therapy on Chronic Periarthritis of Shoulder
摘要
Abstract
Objective:To observe effect of integrated traditional chinese medicine therapy on chronic periarthritis of shoulder and the influence on serum levels of 5-hydroxytryptamine (5-HT) and interleukin-1 β (IL-1 β).Methods:One hundred-forty cases with chronic periarthritis of shoulder were randomly divided into control group (70 cases) and observation group (70 cases).Control group was treated with routine massage.Concrete method:rub Jianyu (LI15),Jianjing (GB21),Jianzhen (S19),Jianzhongshu (SI15),Jianwaishu (SI14),Jianneishu and Ashi heavily,for about 15 minutes;twist and massage scapular region around shoulder joint and upper arm,for about 15 minutes;lift,abduct,rotate outward and adduct the affected limb for 6-8 times,about 10 minutes.Based on control group,cases of observtion group were given with acupuncture and Shentong Zhuyu decoction.For acupuncture treatment,acupoints of Jianliao (TE14),Jianyu (LI15),Jianzhen (S19),Ashi,Geshu (BL17) and Neiguan (PC6) of the affected side are selected.Concrete operations:after acupoints are disinfected in conventional way,puncture the above-mentioned acupoints perpendicularly with disposable sterilized acupuncture needles,with depth at 0.8-1.5 Cun;after having brought about the desired sensation,conduct mild reinforcing-reducing method,with needle retention for 30 minutes during which the needles were manipulated 3 times,each for 3 minutes;acupuncture treatment was performed 5 times per week.Took oral administration of modified Shentong Zhuyu decoction for treatment,and the composition:Duzhong (Eucommia U lmoides) 15 g,Buguzhi (Psoralea corylifolia Linn.) 10 g,Sangjisheng (Loranthus Parasiticus) 10 g,Chuanxiong (Ligusticum Wallichii) 12 g,Taoren (Peach Kernel) 9 g,Honghua (Carthamus Tinctorious) 9 g,Qinjiao (Fraxinus Bungeana) 10g,Qianghuo (Rhizoma seu Radix Notopterygii) 10 g,Moyao (Commiphora Myrrha) 6 g,Danggui (Angelica Sinensis) 9 g,Wulingzhi (Excrementum Pteropi) 6 g,Xiangfu (Rhizoma Cyperi) 6 g,Muxiang (Costustoot) 9 g,N iuxi (Radix Achyranthis B identatae) 10 g,Dilong(Lumbricus) 12 g,Gancao (Radix Liquiritiae) 9 g.One dosage a day,conventional boiling;divided one dose into twice,respectively to be warm taken in the morning and in the evening.1 month was used to observe efficacy of both groups.Scores of joint passive motion functional activity,pain degree,activities of daily living and efficacy were compared between two groups.Serum levels of 5-HT and IL-1 β were detected in both groups.Results:Scores of pain,range of joint passive motion,and functional activities in observation group were remarkably higher,while scores of VAS,PRI,and PPI were lower than those in control group after treatment(P ＜ 0.01).The effective rate of observation group was 95.71％ superior to control group 84.29 (P ＜ 0.05).Serum levels of 5-HT and IL-1 β were remarkably lower than control group after treatment (P ＜ 0.01).Conclusion:Integrated traditional chinese medicine therapy on chronic periarthritis of shoulder can relieve pain,improve joint passive motion functional activity,and increase efficacy,which is related to inhibit serum levels of 5-HT and IL-1β.关键词
