Abstract

Objective To investigate the association of CYP2C19 gene polymorphisms with cerebral infarction induced by transient ischemic attack (TIA) and progression of acute cerebral infarction.Methods A total of 118 patients with TIA who were hospitalized in Emergency Department and Department of Neurology in Zaozhuang Municipal Hospital from March 2015 to September 2016 were enrolled,and according to the presence or absence of cerebral infarction within 90 days,these patients were divided into cerebral infarction group (group A) with 28 patients and non-cerebral infarction group (group B) with 90 patients.A total of 179 patients with acute cerebral infarction who were treated during the same period of time were enrolled,and according to the presence or absence of progression,these patients were divided into cerebral infarction progression group (group C) with 44 patients and non-cerebral infarction progression group (group D) with 135 patients.Fluorescence in-situ hybridization was used to determine the genotype of CYP2C19.Results Compared with group B,group A had a significantly lower constituent ratio of extensive metabolizers (x 2 =10.619,P < 0.01) and a significantly higher constituent ratio of poor metabolizers (x 2 =10.453,P < 0.01).Compared with group D,group C had a significantly lower constituent ratio of extensive metabolizers (x2 =13.846,P < 0.01) and a significantly higher constituent ratio of poor metabolizers (x2 =11.238,P<0.01).The multivariate logistic regression analysis showed that CYP2C19 gene polymorphisms were independently associated with cerebral infarction induced by TIA (OR =2.013,95% CI=1.117-2.551,P<0.05) and the progression of acute cerebral infarction (OR =1.875,95% CI =1.193-2.461,P<0.05).Conclusion TIA patients who are extensive metabolizers in the CYP2C19 gene have a low risk of cerebral infarction within 90 days,and those who are poor metabolizers tend to develop cerebral infarction.Patients with acute cerebral infarction who are poor metabolizers in the CYP2C19 gene tend to experience progression,while those who are extensive metabolizers have a low risk of progression.
