分级诊疗背景下社区老年冠心病患者医养结合健康管理模式研究
Mode of Healthcare Management Services Delivered by Medical and Elderly Care Institutions for Community-dwelling Elderly Patients with Coronary Heart Disease under the Situation of the Implementation of Hierarchical Medical System in China

Abstract
Objective To investigate the effect of mode of healthcare management services delivered by medical and elderly care institutions on the self-management ability and quality of life in community-dwelling elderly patients with coronary heart disease under the situation of the implementation of hierarchical medical system in China.Methods Eighty cases of elderly patients with coronary heart disease treated in Nanfang Hospital,Southern Medical University,from March 2013 to May 2015 were selected as the subjects,and divided into the control group and study group according to the random digital table with 40 cases in each.Patients in the control group received 6-month conventional mode of healthcare management services,those in the study group received 6-month conventional mode combined with mode of healthcare management services delivered by medical and elderly care institutions.Before and after intervention,the self-management ability and quality of life were assessed,and major indicators of cardiac function including left ventricular end-diastolic diameter (LVEDd),stroke volume (SV),ejection fraction (EF) were measured in both groups.And the patients' satisfaction with the effect of intervention was assessed in both groups after intervention.Results After intervention,the scores of first aid management,life management,diet management,medication management,bad habits management,exercise management,emotional management in the study group were higher than those in the control group (P ＜ 0.05),and the scores of physical functioning,physical role functioning,body pain,general health perceptions,vitality,social role functioning,emotional role functioning,mental health in the study group were significantly higher than those in the control group (P ＜ 0.05);the study group had lower LVEDd but higher SV and EF than the control group (P ＜ 0.05).Significantly higher patients' satisfaction with the effect of intervention was noted in the study group than in the control group (Z =6.23,P =0.01).Conclusion By using the mode of healthcare management services delivered by medical and elderly care institutions under the situation of the implementation of hierarchical medical system in China,the self -management ability and quality of life in community-dwelling elderly patients with coronary heart disease can be effectively improved,and high satisfaction with the services can be achieved,so the mode is worthy of promotion and application.关键词
