64排螺旋CT血管成像对于急性肠系膜上动脉栓塞的诊断价值

Abstract
Objective: To explore the diagnostic value of multi-slice CT (MSCT) angiography in the acute superior mesenteric artery embolism(ASMAE). Methods: The clinical data of 16 patients with ASMAE were analyzed by using retrospective analysis. And they were divided into conservative treatment group(A group, 4 cases), thrombectomy group (B group, 5 cases) and combined group (thrombectomy and intestinal canal excision)(C group, 7 cases). And the position of SMAE, development amount of I grade branch of SMA, target sign of intestinal tube, thinned intestinal wall, dilatation of intestine, pneumatosis of intestinal wall, mesenteric stranding and ascites of MSCT among three groups were compared. Besides, the CT value of ischemic intestinal tube was measured. Results: The development amount of I grade branch of SMA and CT value of ischemic intestinal tube in arterial phase of A group were significantly higher than that of B group and C group, respectively. And the development amount of C group was least, and its CT value of ischemic intestinal tube in arterial phase was lowest. And the pneumatosis of intestinal wall of C group was significantly larger than that of other two groups. The differences of development amount and CT value among the three groups were statistically significant (F=14.34, F=28.91, P<0.05), respectively. As the anatomical region of Fullen, most of thrombus position of C group were at the proximal mouth of inferior pancreaticoduodenal artery(I anatomical region) and between the proximal mouth of inferior pancreaticoduodenal artery and arteria colica media (II anatomical region), and the difference of thrombus position between different regions were significant (F=9.51, P<0.05). Conclusion: MSCTA not only can directly reflect the situation of SMA in ASMAE, but also precaution the ischemic degree of intestinal wall. So it might provide reliable image support for treatment decision of clinical doctor.关键词
