妇科腹腔镜手术中转开腹的原因分析
Cause analysis of conversion from laparoscopic gynecologic operation to laparotomy

Abstract
Objective:To analyze the common causes of converting to laparotomy during gynecological laparoscopic surgery and discuss the preventive measures in order to reduce operative complications and ensure patients ' perioperative safety .Methods:Clinical data of 60 cases of conversions to open surgery out of 5033 cases of gynecological laparoscopic operation from Jan .2010 to Jun.2016 were analyzed retrospectively .Results:The conversion rate was 1.19% in 5033 cases.It was 2.03% in the early time ( Jan.2010-Mar.2013) and 0.53%in the later (Apr.2013-Jun.2016).The primal causes of conversion to laparotomy included severe pelvic ad-hesion (50%),special location of the myoma (16.7%) and adenomyoma (11.7%).60 patients who were converted to open surgery all achieved desired results ,were cured without any complications and discharged uneventfully .Conclusions:The conversion to laparoto-my during gynecological laparoscopic surgery should be considered as a wise decision which ensures patients ' safety and prevents seri-ous complications rather than an operative complication .The conversion rate could be significantly reduced by preoperative pelvic pre-cise evaluation ,strict mastering of operative indication and skillful operative technique .关键词
