Abstract
Objective To investigate the effect of gestational weight gain and body mass index(BMI)in third trimester of pregnancy on postpartum lactation.Methods Totally 623 puerperas delivering in Wenzhou Central Hospital from July 2012 to September 2015 were selected,and they were divided into group A(lactation volume > 45mL,lactation initiating time ≤ 48h)and group B (lactation volume≤45mL,lactation initiating time>48h).Two groups were compared in 24h prolactin after maternity,lactation volume and initiating rate of lactation,and clinical data of two groups were analyzed by univariate and multivariate Logistic regression analysis.Results In group A,the prolactin and lactation volume were significantly higher than those in group B(t value was 2.357 and 2.093,respectively,both P<0.05),and the initiating rate of lactation was significantly higher than that of group B (χ2= 6.108,P< 0.05).There were significant differences in maternal age,gestational age,birth childbirth way,BMI in late pregnancy,gestational weight gain,gestational hypertension disease,and gestational diabetes between two groups(χ2value ranged 3.893-8.324,respectively,all P<0.05),but the differences in parity and neonatal weight between two groups were not significant(χ2value was 0.687 and 2.451,respectively,all P> 0.05).Factors those were statistically significant in univariate analysis were analyzed in multiple factors Logistic regression analysis,and the results showed that old maternal age,high BMI in late pregnancy and excessive gestational weight gain were independent risk factors influencing maternal lactation(OR value was 2.942,2.012 and 3.522,respectively,all P<0.05).Conclusion High BMI in late pregnancy and excessive gestational weight gain are independent risk factors for lactation,so maternal exercise during pregnancy should be done to avoid obesity.关键词
