摘要

Abstract

Objective To investigate the effect of jet nebulizer therapy on ventilator performance during noninvasive positive pressure ventilation of four noninvasive ventilators.Methods The Active Servo Lung 5000 respiratory simulation system (ASL5000) was used to simulate a COPD patient.Nebulizer positions were set to be near the distal or proximal end of the exhalation valve.The driving flow for nebulization was set at 4 L/minor 8 L/min.The effect of jet nebulizer therapy on ventilator performance,such as trigger,control and tidal volume,were compared during noninvasive positive pressure ventilation under different experimental conditions.Results No instances of auto-triggering or missed triggers of the ventilator were observed under all experimental conditions.Nebulization did not affect peak inspiratory pressure,peak flow,mean flow inspiratory time or tidal volume displayed on simulated lung,which was equivalent to the actual tidal volume of the patient.Under certain experimental conditions nebulization significantly affected time to trigger,trigger work,the magnitude of airway pressure drop needed to trigger,time from ventilator triggering to airway pressure achieved 90% of the maximal pressure level during inspiration and tidal volume displayed on ventilator.Position of nebulizer and driving flow also had significant effect on ventilator performance.Condusion Jet nebulizer therapy,has significant effect on ventilator performance during noninvasive positive pressure ventilation under certain experimental conditions.
