宝石能谱CT在鉴别良恶性胸腔积液中的应用价值
Application value of spectral CT in differential diagnosis of benign and malignant pleural effusion

Abstract
Objective:To investigate the value of spectral CT in the differential diagnosis of benign and malignant pleural effu-sion. Methods:14 patients with benign pleural effusion(8 with pneumonia,5 with pulmonary tuberculosis and 1 with pneumocys-tosis)and 15 patients with malignant pleural effusion(5 with adenocarcinoma of lung,4 with squamous cell carcinoma,2 with adenosquamous carcinoma and 4 with metastatic carcinoma)underwent non-contrast spectral CT imaging. All patients were con-firmed by pathology. Various parameters for the effusion were measured and compared:CT values of GSI QC,the slope of the spectral curve,monochromatic images at 40 keV and 100 keV,the material contents on the water-,fat- and blood-based material decomposition images,and the effective atomic number Z. Results:The CT values of GSI QC,the slope of the spectral curve,the water-density,fat-density and blood-density between benign and malignant pleural effusion had no differences(all P>0.05). CT values of monochromatic 40 keV and 100 keV spectral images and the effective atomic number Z between benign and malig-nant pleural effusion were statistically different(all P<0.05). Conclusion:The spectral CT provides CT value measurements at both high and low energy levels and effective atomic number to assist the differential diagnosis of benign and malignant pleural effusion.关键词
