Abstract

Objective To observe the efficacy and safety of the " air decompression" syringe irrigation in flexible ureteroscopic lithotripsy with holmium laser.Methods From September to December in 2016,a total of 100 patients with upper urinary calculi who accepted flexible ureteroscopic lithotripsy treatment in the Second Affiliated Hospital of Zhengzhou University were divided into group A and group B by random number table method.50 cases in group A were treated by artificial syringe irrigation,and 50 cases in group B were treated by " air decompression" syringe irrigation.The postoperative body temperature >38.5 ℃,white blood count (WBC) >10×l09/L,procalcitonin (PCT) >0.1 μg/L,PCT>0.5 μg/L,and systemic inflammatory response syndrome (SIRS) were observed in the two groups.The intraoperative renal pelvic pressure was monitored in patients with the presence of renal fistula channel.Results There were no statistically significant differences in the incidence of postoperative body temperature > 38.5 ℃,PCT > 0.5 μg/L and SIRS in the two groups (P > 0.05).The incidence of WBC > 10 × 109/L and PCT > 0.1 μg/L in group B were lower than those in group A (P < 0.05).25 patients with fever after operation were discharged from hospital after empiric antibiotic treatment.The intraoperative pulse water injection curve was observaed:the instantaneous perfusion pressure in group A showed a sudden rise curve,group B showed a slow upward trend.Conclusion " Air decompression" syringe irrigation can keep the renal pelvis pressure relatively stable,improve the sudden change of instantaneous perfusion rate and perfusion pressure caused by the difference of the artificial syringe irrigation.It is safe and effective in flexible ureteroscopic lithotripsy with holmium laser,and is especially suitable for high-risk groups who are susceptible to infection.
