Abstract
Objective To evaluate the impact of obesity on hospital mortality in mechanically ventilated ICU patients.Methods We enrolled 210 consecutive inpatients who received treatment in General ICU,North China University of Science and Technology Affiliated Hospital between January and December 2015,and stratified them into the obese group (n =84) and no-obese group (n =126) based on the criteria for the assessment of obesity in Chinese adults.The follow-up period ended at the date of death or at the end of 60-day treament in the ICU.Outcome indicators included ICU mortality,hospital mortality,mortality after receiving 60-day treatment in the ICU,duration of mechanical ventilation (MV),length of stay in the ICU,length of stay,and incidence of complications (duration of MV > 21 days,re-intubation,tracheotomy,ventilator-associated pneumonia,central venous catheter infection,deep venous thrombosis and pressure ulcers).The influence of BMI on hospital mortality was analyzed.Results No patients were lost to follow up,dropped out of the study or had data loss.The ICU mortality,hospital mortality,mortality after receiving 60-day treatment in the ICU,re-intubation rate,tracheotomy rate,ventilator-associated pneumonia rate,central venous catheter infection rate and deep venous thrombosis rate did not differ significantly between the two groups (P > 0.05).Patients in obese group had longer duration of MV,longer length of stay in the ICU and length of stay than those in the non-obese group (P < 0.05).More obese group patients were found with MV for more than 21 days compared with non-obese group ones (P < 0.05).No pressure ulcer occurred in all patients.BMI was not an independent risk factor for hospital mortality in mechanically ventilated patients [B =0.041,SE =0.035,OR =1.048,95 % CI (0.982,1.118),P =0.158].Conclusion Obesity is not an independent risk factor for hospital mortality in mechanically ventilated ICU patients,but it is associated with prolonged duration of MV,increased length of stay in the ICU and length of stay.关键词
