Dynamic Complexity Analysis of the Community-based Colorectal Cancer Screening Program Based on System Dynamics
摘要
Abstract
Objective To investigate the dynamic complexity of the community-based colorectal cancer screening program based on the analysis of a reprehensive sample,the colorectal cancer screening program applied to Gumei Community Health Service Center located in Minhang District of Shanghai.Methods From May to August 2016,we collected the historical and implementation data of the community-based colorectal cancer screening program performed by Gumei Community Health Service Center during 2008-2016,carried out focus group interview with the key persons for the implementation of the program,and based on the above data,constructed a system model of the program and workflow charts of its subsystems with system dynamics modeling software Vensim 5.6.Results The colorectal cancer screening program carried out in Gumei Community comprises governmental surveillance and assessment mechanism subsystem,community-based annual colorectal cancer screening plan and execution subsystem,colorectal cancer screening subsystem as well as information management subsystem for people identified with colorectal lesions.These 4 subsystems interact with each other through budgeting,executing and assessing.Analysis of the relationship between variables in one subsystem or among subsystems showed that,the crucial factors for achieving successful screening were health education,initially screened population and the number of people undergoing colonoscopy.Conclusion Using system dynamics to explore the dynamic complexity of the community-based colorectal cancer screening program is helpful to detect the causal relationship between the variables in the screening,identify the crucial factors for achieving successful screening and the challenges during the screening,by which the outcome of community-based colorectal cancer screening program can be improved.关键词
