原发性高血压患者24小时尿钠钾排泄比与左心室质量指数的相关性研究
Relationship of 24-hour Urinary Sodium-to-potassium Excretion Ratio with Left Ventricular Mass Index in Essential Hypertensive Patients

Abstract
Objective To explore the relationship of 24-hour urinary sodium,urinary potassium and urinary sodium -to-potassium excretion ratio with left ventricular mass index (LVMI) in essential hypertensive patients.Methods The enrolled participants were 293 consecutive patients with essential hypertension who received inpatient treatment in Department of Cardiovascular,the First Affiliated Hospital of Xinjiang Medical University from 2015 to 2016.24-hour urine specimens were collected to detect the level of 24-hour urinary sodium and urinary potassium and urinary sodium-to-potassium excretion ratio was calculated.According to the concentrations of 24-hour urinary sodium,the patients were divided into three groups:group A (urinary sodium≤100 mmol/24 h),group B (100 mmol/24 h < urinary sodium ≤200 mmoL/24 h) and group C (urinary sodium > 200 mmol/24 h).Echocardiography was performed in all participants,and left ventricular end-diastolic diameter (LVEDD),left ventricular end-systolic diameter (LVESD),interventricular septal thickness (IVST),left ventricular posterior wall thickness (LVPWT),left ventricular ejection fraction (LVEF) and LVMI were recorded.Results The values of IVST and LVPWT differed significantly between the three groups (P < 0.05).Specifically,group C had higher value of IVST than the other groups (P < 0.05),and it had higher value of LVPWT than group A (P < 0.05).Spearman correlation analysis showed that LVMI was positively correlated with 24-hour urinary sodium-to-potassium excretion ratio (rs =0.119,P <0.05),but it did not correlate with the level of 24-hour urinary sodium and urinary potassium (rx =0.139,0.087,P >0.05).Multiple linear regression analysis showed that 24-hour urinary sodium-to-potassium excretion ratio was positively correlated with LVMI [β (95% CI) =1.287 (0.116,2.457),P < 0.05],while the concentrations of 24-hour urinary sodium [β (95%CI) =-0.011 (-0.050,0.027),P >0.05] and 24-hour urinary potassium [β (95% CI) =0.071 (-0.096,0.237),P > 0.05] were not associated with LVMI.Conclusion The 24-hour urinary sodium-to-potassium extraction ratio is associated with LVMI,which is independent of blood pressure.关键词
