Abstract
Background Both multidetector CT (MDCT) and magnetic resonance imaging (MRI) are commonly used in clinical diagnosis of extrahepatic bile duct stones,but each of them has its own advantages and disadvantages.Objective To compare and analyze the diagnostic value of MDCT and 3.0T MRI for extrahepatic bile duct stones,in order to offer suggestions for clinical selection of appropriate imaging method for patients with suspected biliary calculus.Methods Fifty-six cases with suspected extrahepatic bile duct stones treated in the Second Hospital of Hebei Medical University from 2009-06-18 to 2014-10-21,were examined by MDCT and 3.0T MRI in 2 weeks before endoscopic retrograde pancreatic cholangiography (ERCP).ERCP results were set as the gold standard for assessing the diagnostic value of MDCT and 3.0T MRI.The sensitivity,missed diagnosis rate,specificity,misdiagnosis rate and other indicators of MDCT and 3.0T MRI were calculated.Results For the diagnosis of extrahepatic bile duct stones,the sensitivity,false negative rate (missed diagnosis rate),specificity,false positive rate (misdiagnosis rate),positive predictive value and negative predictive value were 25.6％,74.4％,100.0％,0,100.0％,28.9％,respectively for MDCT,and 69.8％,30.2％,100.0％,0,100.0％,50.0％,respectively for 3.0T MRI.Compared with MDCT,3.0T MRI presented higher sensitivity and lower missed diagnosis rate (P ＜ 0.001),but the specificity and misdiagnosis rate of the two methods were the same.Conclusion MRI examination may be recommended for patients with suspected extrahepatic bile duct stones if conditions permit.关键词
